Why Are Medicare Supplement Plans So Expensive in CA, FL, and NY?
California, Florida, and New York have some of the highest Medigap premiums in the country, and here’s why:
- Larger Medicare populations drive demand and increase costs.
- Higher healthcare costs across the board.
- Consumer-friendly state rules, like:
- New York: No health underwriting ever.
- California: The Birthday Rule lets you switch Medigap plans annually—no health questions.
While these protections are fantastic for beneficiaries, they also lead to higher monthly premiums because insurance companies assume more risk.
Quick Refresher: Plan G vs. Plan N
Plan G – Full Coverage Minus the Deductible
- Covers everything Original Medicare doesn’t—except the Part B deductible, which is $283 in 2026.
- No copays, no coinsurance, and no excess charges.
- Use any doctor nationwide who accepts Medicare.
In high-cost states, Plan G can run $150 to $300/month for a 65-year-old. It’s ideal for someone who:
- Visits doctors regularly
- Has ongoing health conditions
- Wants complete peace of mind and zero billing surprises
Plan N – Smart Savings with Some Trade-Offs
- Also covers most gaps in Original Medicare.
- Copays: Up to $20 for office visits, up to $50 for ER visits (waived if admitted).
- Does not cover Part B excess charges—rare but possible depending on your provider.
In CA, FL, and NY, Plan N typically costs $150–$200/month, which is $30–$60/month less than Plan G.
It’s a great fit if you:
- Are in good health
- Don’t mind small copays
- Want to lower your monthly premium and are okay with minimal cost-sharing
Real Example: Saving Over $1,000/Year with Plan N
Just last week, we helped a client in New York who was paying $300/month for Plan G. She saw her doctor only a few times a year, so we recommended switching to Plan N at $210/month.
Now she:
- Saves over $1,000 a year
- Still sees any Medicare doctor she wants
- Pays a small co-pay only when she visits the doctor
And because she lives in New York—where no medical underwriting is required—she can switch back to Plan G any time if her health needs change.
The Truth About Excess Charges
One concern people have with Plan N is excess charges, which Plan G fully covers.
Here’s what you need to know:
- Only about 5% of Medicare providers bill excess charges.
- In some states—like New York, Connecticut, and Massachusetts—excess charges are banned entirely.
- In California and Florida, they’re allowed but still rare.
So if you’re okay with a small risk and screen your providers, Plan N could save you a significant amount—especially in higher-cost states.
What’s Best for You Depends on Usage and Budget
Choose Plan G if:
- You want no copays and zero surprise bills
- You see multiple doctors or specialists regularly
- You prefer maximum simplicity and predictability
Choose Plan N if:
- You’re in good or excellent health
- You want to save $30–$60/month
- You don’t mind occasional copays
- You live in a state with excess charge protections (like NY)
- You value freedom to switch plans under your state’s rules
Flexibility Is Key in High-Cost States
If you live in:
- New York: You can switch Medigap plans anytime—no health questions required.
- California: Use the Birthday Rule to switch plans without underwriting once a year.
- Florida: No special switching rules, but plenty of carrier options to compare.
That means you’re not locked in forever. You can try Plan N, see how it works, and if your needs change, switch to Plan G later—especially in NY or CA where rules are more lenient.
What If You Haven’t Enrolled Yet?
If you’re turning 65 soon or newly eligible for Medicare:
- Compare Plan G and N side-by-side.
- Evaluate your current health, doctor visit frequency, and budget.
- Don’t just go with what your friend or neighbor has—choose the plan that aligns with your needs.
Remember, the goal is to maximize your coverage and minimize your cost. In high-cost states, Plan N often wins on savings if you’re healthy, while Plan G wins on simplicity if you want to avoid co-pays altogether.
